Savannah Ray, MSN, FNP-C, supports Providers Health Solutions as a healthcare executive and board-certified family nurse practitioner with more than 15 years of experience in population health, clinical quality, and operations. She has developed multistate population health programs, led teams of more than 70 clinical staff, and advanced care coordination, transitional care, wellness, quality improvement, and cost reduction. Her population health experience includes programs developed in partnership with Vantage Health Plan. At PHS, Savannah brings a clinical and operational perspective to medical management, member engagement, quality strategy, care navigation, and programs for high-risk or underserved populations. Her work helps ensure that self-funded cost strategies remain grounded in appropriate care, practical implementation, and measurable health outcomes.
